Personal Care Services

We offer state-approved Personal Care Services for patients who qualify for Medicaid. Personal Care includes in-home assistance with Activities of Daily Living which include bathing, dressing, grooming and meal prep, as well as, light housekeeping. Your personalized plan of care will depend on your state assessment, which is performed by Liberty Healthcare. We work with you, your family, Liberty Healthcare and your doctor to meet your individual needs.
