CAP Services

We offer state-approved CAP services for patients who qualify for Medicaid. CAP Services include in-home assistance with Activities of Daily Living which include bathing, dressing, grooming and meal prep, as well as, light housekeeping. In addition, we offer to help with errands such as grocery pick-up, pharmacy pick-up, personal banking or general transportation. Your personalized plan of care will depend on your CAP assessment, which is performed by your local CAP lead agency. We work with you, your family, your CAP case manager and your doctor to meet your individual needs.
